Rattie Ratz's Adoption Contract

Rattie Ratz's Adoption Contract

This is just the "contract" section of the form so that you know what to expect to agree to when you choose to adopt from Rattie Ratz.

This Adoption Contract (hereinafter referred to as “the Contract”) is entered into by and between the above listed Adopter (hereinafter referred to as “the Adopter”) and the above listed Adoption Agency and its agents, including but not limited to employees and volunteers, collectively known as Rattie Ratz (hereinafter referred to as “the Agency”) concerning the adoption of the above-described animal (hereinafter referred to as “the Animal”).

The Adopter and the Agency agree that the adopter:
(1) Will care for the animal in a humane manner.
(2) Will keep the animal as a household pet in the Adopter’s own home.
(3) Will provide adequate food, water, and other supplies (e.g., shelter, bedding, exercise wheel) as needed.
(4) Will provide care, affection and attention for the animal.
(5) Will promptly obtain veterinary care if the animal has an ailment, injury, or illness that Adopter is not able to care for properly.
(6) Will not allow the animal to breed, be used for testing or laboratory purposes, or be used for vivisection, or in any other inhumane manner.
(7) Will not abandon, sell, give away, dispose of, turn over to, or relinquish custody or possession of the animal except to return the animal to the Agency or with the approval of the agency.

The Adopter further agrees that if, for any reason, the Adopter does not wish to keep the animal or if the Adopter cannot abide by all of the terms of this Contract, the Adopter will promptly contact the Agency and will seek no funds from the Agency as reimbursement, or otherwise.

The Adopter declares that the Adopter is aware:
(1) That animals are different from human beings in their response to human actions, so the agency can not guarantee compatibility with the adopter, end user, environment or supplies, or other animals.
(2) That the actions of animals are often unpredictable and the possibility of bites, scratches, injury to the animal due to it’s environment, etc. exists.
(3) That the Agency makes no claims or representations as to the animal or as to the condition, suitability for use, behavior or temperament of the animal put up for adoption. The animal is transferred to the Adopter as is. This supercedes any and all verbal representation made by the agency.
(4) That the Agency is not responsible for any illness or injury that now exists or hereafter occurs, for any treatment given, or for any medical costs incurred once adopted.

The Adopter accepts possession and responsibility of and for the animal and accompanying supplies (if any) including but not limited to cages and food, in consideration hereof, and at the Adopter’s own risk, and hereby covenants and agrees not to sue or assert any claim, demand or action against or with respect to the Agency for the Adopter and others directly or indirectly regarding or arising from the animal. The Adopter hereby holds harmless and agrees to indemnify and defend the Agency from any rights and claims against Agency which Adopter or others now or in the future have or may have for any damages or injuries to person or property caused by or with respect to said animal or accompanying supplies (if any) and the Adopter and others, release and waive any and all such rights and claims. The Adopter understands and agrees that the adopter has the right to do this and that this is furnished in favor of the Agency, its officers, directors, members, volunteers, employees and agents respectively.

The Agency, as a continuing condition of the Contract and obligation resulting from the adoption of the above described animal from the Agency, agrees that the Agency:
(1) Will accept the animal back at any time for any reason.
(2) Will remain available to the Adopter for ongoing advice as needed regarding the animal.
(3) Will accept $_________ as a donation for the adoption of the animal(s) by the Adopter.

The information provided to the Adopter concerning the animal’s description, history and condition is complete and correct to the best of Agency’s knowledge and belief. The Agency recommends that a licensed Veterinarian examine the animal within 72 hours of its adoption.

The undersigned attests the information provided herein is true and correct to the best of their knowledge and belief and agree with the terms contained within.
